As a new client, we will need some basic information about you and your pet. Please fill out our new client form, located below, at your convenience in the comfort of your own home. Having this form in advance lets us record our patients and owners into our management software so that we can effectively and efficiently provide the service you deserve!
Due to the amount of time our staff and doctors devote to preparing and reserving uninterrupted time for your pet’s appointment, we require a deposit to reserve your appointment. We will work hard to accommodate an appointment that fits your schedule and your pet’s needs. We ask that you let us know about changes 24-hours in advance. We understand life happens, but without the 24-hour notice, your deposit will not be refunded.
There is limited availability in our schedule to accommodate new clients therefore, same-day appointments are not usually available. Appointments need to be made in advance and if your pet is experiencing an emergency, please contact one of the local emergency hospitals Bluepearl Tacoma, Summit Veterinary Referral Center, or Bluepearl Renton.
